This Guide is issued by SW Project Consulting Sdn Bhd to assist Employer who applied HRDC Grant to furnish the documents. There are three documents to be prepared by SWPC Project Consulting Sdn Bhd and to be acknowledged by the Employer who applied HRDC Grant.:
- JD14 - Joint Declaration Form
- JD14 Form only can be approved by Manager and Director.
- JD14 Form needs to prepare according to the information as per image below.
- T301 - Training Attendance Form
- T301 Form must show correct information as per grant application (Full Name of Trainee and MyKad Number)
- T301 Form needs to prepare according to the information as per image below
- System Generated Attendance Report - Generated by ZOOM after Webinar
- System Generated Attendance Report must be signed and stamped
- System Generated Attendance Report needs to prepare according to the information as per image below
Digital Signature - Please do not use Digital Signature and Digital Stamp.
